In A Single Gene, A Path To Fight Heart Attacks |
Two major studies by leading research groups published on Wednesday independently identified mutations in a single gene that protect against heart attacks by keeping levels of triglycerides — a kind of fat in the blood — very low for a lifetime. These findings are expected to lead to a push to develop drugs that mimic the effect of the mutations, potentially offering the first new class of drugs to combat heart disease in decades, experts say. Statins, which reduce LDL cholesterol, another cause of heart disease, became blockbusters in the late 1980s. Since then there have been no major new drugs approved for lowering heart disease risk. But experts caution that drug development takes years and that there are no guarantees that new treatments will work as hoped.
Heart attacks are the leading killer in the United States, and about 720,000 Americans a year have them.
Although statins are effective in reducing heart attack risk, many users still often have high levels of triglycerides and go on to have heart attacks. So the results of the new studies are good news, said Dr Daniel J Rader, the director of the Preventive Cardiovascular Medicine and Lipid Clinic at the University of Pennsylvania, who was not involved in the research. "We've been looking for something beyond statins," Rader said. "After we have put people on high-dose statins, what else can we do? Essentially nothing."
Experts differ in their estimates of how many Americans might be candidates for a triglyceride-lowering drug. If the eligible group included all adults with triglyceride levels of 200 or more — the normal level is 150 or less — that would mean about 20 percent of adult Americans. If it was just those with the highest levels, above 500, then 2 percent to 3 percent of adults would qualify. The discovery announced on Wednesday was hinted at in 2008 in a much smaller study in the Amish conducted by researchers from the University of Maryland's medical school. One in 20 Amish people has a mutation that destroys a gene, APOC3, involved in triglyceride metabolism, as compared with one in 150 Americans generally. The scientists were intrigued but did not have enough data to nail down the gene's role in heart attacks.
Sam, a 55-year-old Amish farmer who declined to have his last name published, saying he was uncomfortable about being conspicuous, has such a beneficial mutation. He recalls little heart disease in his family. On a cold day last fall, as an icy rain fell outside, he sat at a small wooden table in his daughter's house and laid out a sheet of paper that showed he had a triglyceride level of 45. The average in the United States is 147. "It's nice that something came out that is positive," he said. Triglycerides have long puzzled researchers, although they are routinely measured along with cholesterol in blood tests and are often high in people with heart disease. Many experts were unconvinced they actually caused heart attacks. Clinical trials of drugs that lowered triglycerides by a small amount added to doubts about their role in heart attacks. The drugs had no effect on heart attack rates.
As for triglycerides themselves, "do they just keep bad company or are they independently doing something to risk?" asked Dr Robert Hegele, a heart disease expert at Western University in London, Ontario, who was not involved with the new studies. Those studies, published in The New England Journal of Medicine and funded by the National Institutes of Health and the European Union, provide "a very, very strong type of evidence," Hegele said, that triglycerides are in fact a cause of heart attacks.
The work began several years ago when researchers at the Broad Institute of Harvard and MIT starting searching through an enormous data set drawn from 70 studies involving 200,000 people to see if there were tiny genetic changes near or in genes that seemed to lead to very high or very low amounts of triglycerides in the blood. They also asked whether people who happened to have a higher or lower triglyceride level also had a higher or lower incidence of heart attacks. The researchers discovered that people with a genetic predisposition to higher triglyceride levels had more heart attacks and those with genetically lower triglyceride levels had fewer. Their study, published last year in Nature Genetics, did not isolate individual genes, though. It just pointed to signposts on the long stretch of 30 million DNA letters that were near the genes. So the investigators began a hunt for the genes themselves.
To that end, they mapped the genes of 3,734 Americans, about 2,500 of whom were white and the balance African-American. The researchers reported the results of that effort on Wednesday. One gene, APOC3, stood out. The scientists found four mutations that destroyed the function of this gene. The Amish study had discovered that people with such a mutation could drink a big rich milkshake, loaded with fat, and their triglyceride levels did not budge. For everyone else, they spiked. The new studies show what that means to people's health. "Those who carry the gene mutations have a 40 per cent reduction in triglyceride levels and a 40 per cent lower risk of heart disease," said Dr Sekar Kathiresan of Massachusetts General Hospital and the Broad Institute. He is the lead researcher on the gene project.
Now, he added, "there is a route to heart attacks that is independent of LDL," referring to the form of cholesterol associated with heart disease. The other study, led by Dr Anne Tybjaerg-Hansen of Copenhagen University Hospital, used data from 75,725 subjects to learn whether low triglyceride levels were linked to a reduced heart attack risk. They were. The researchers also asked whether people who had mutations destroying the APOC3 gene had fewer heart attacks. They did. Those with such mutations had a 44 percent reduction in triglycerides and a 36 per cent lower heart attack risk. The Danish data, Kathiresan said, "are eerily consistent with our data."
"It is incredible how reproducible the finding is," he added. Yet the two groups worked independently, with The New England Journal of Medicine coordinating publication of their studies. For the University of Maryland scientists who conducted the 2008 Amish study, the new finding shows that their hypothesis about the gene was correct. Those with the mutation had less calcification of their arteries, an indication that they had less heart disease. But the Maryland group was not able to say if those people also had fewer deaths from heart disease. Toni Pollin, lead researcher for the study, explained: "To really find out if it is related to heart disease we'd need to follow people over time. And the Amish use hospitals a lot less than other people, so it is hard to document the cause of death." Meanwhile, a small California company, Isis, also hit upon the gene when it was looking for ways to make triglyceride levels plunge in the small group of people with disorders leading to triglyceride levels so high they can be fatal. They made a drug that counteracts the gene and began testing it. It slashed triglyceride levels by 71 per cent.
"It is the most important drug in our pipeline," said Dr Stanley Crooke, the chief executive of Isis. "Trust me, these data are really exciting." The company has no plans to test whether its drug prevents heart attacks in the general population. That sort of huge study, lasting years, would require the resources of a much larger company. But heart researchers see the Isis drug as, at least, proof that it is possible to come up with drugs in what could be a new class of pharmaceuticals to protect against heart disease.

High consumption of sugar sweetened drinks, i.e. an average of 2.7 per day in healthy, thin men was associated with 6.3 percent lowered sperm motility (ability of sperm to move toward egg) compared to those who reported taking less than a drink daily. Researchers couldn't find such results in obese or overweight men. Additionally, men who reported drinking more sugary drink, both lean and obese men had reduced levels of a reproductive hormone follicle stimulating hormone (FSH). FSH plays a major role in regulation of sperm production.

Middle-aged people who eat protein-rich food are four times more likely to die of cancer than someone who only eats a little, according to a new study. The researchers said eating a lot of protein increased the risk of cancer almost as much as smoking 20 cigarettes a day. They reached their findings, published in the journal Cell: Metabolism, after tracking thousands of people over 20 years. "We provide convincing evidence that a high-protein diet — particularly if the proteins are derived from animals — is nearly as bad as smoking for your health," one of the academics behind the work, Dr Valter Longo, of the University of Southern California, told Daily Telegraph. A high-protein diet was defined as one in which 20 per cent of the calories came from protein. They recommended eating 0.8g of protein per kilogram of body weight a day during middle age.
However, the researchers said protein had benefits during later life. Dr Eileen Crimmins, a co-author of the study, said: "We also propose that at older ages, it may be important to avoid a low-protein diet to allow the maintenance of healthy weight and protection from frailty." However Dr Gunter Kuhnle, a food nutrition scientist at the University of Reading, criticized the study for making a link to smoking. "While this study raises some interesting perspectives on links between protein intake and mortality ... It is wrong, and potentially even dangerous, to compare the effects of smoking with the effect of meat and cheese," he said.
"The smoker thinks: 'Why bother quitting smoking if my cheese and ham sandwich is just as bad for me?'"
And Professor Tim Key, of Cancer Research UK, said: "Further research is needed to establish whether there is any link between eating a high protein diet and an increased risk of middle aged people dying from cancer."

Activist wants to Sell Condoms at Bus Stops to Promote Women’s Health |
Think of a hawker carrying a belly tray at a busy traffic junction, a bus station or in a cricket stadium and peddling not sweets and snacks, but female contraceptives! India may not be ready for it now, but the innovative concept has already sparked huge interest in a number of countries, Beatrijs Janssen, a healthcare expert from the Netherlands, says. "I have already demonstrated it for promotion at conferences in several countries," Janssen says. Being a woman, she knows the importance of female condoms to ward off not just unwanted pregnancies, but also the menace of deadly HIV/AIDS.
Unsurprisingly, she has become the cynosure of hundreds of healthcare experts gathered at the ongoing first Global Health Conference on Social Marketing and Social Franchising, organised by HLFPPT(Hindustan Latex Family Planning Promotion Trust), a not-for-profit trust promoted by the mini-Ratna public sector enterprise HLL Lifecare Ltd, here. Janssen is the Communication Advisor to Universal Access to Female Condoms (UAFC), a joint programme launched in 2009 by four organisations (Oxfam Novib, Rutgers WPF, i+solutions and the Netherlands Ministry of Foreign Affairs) with the aim of making female condoms accessible and affordable for all. The Dutch health activist is serious about popularising the novel concept. To promote the idea and create awareness, she, along with other activists, has been working around the globe for the last four month, carrying the belly tray laden with a range of female condoms. India can take pride in the fact that the contraceptives on the tray include the ones manufactured by HLL Lifecare Ltd. "I am not sure about India, but it can soon be a reality as a marketing tool and a way of selling condoms. We have to modify these trays as per the preferences of the local people," Janssen says. "We thought about this idea, just like the system of selling goodies at football stands. The prime focus is to have mobility, instead of a stationary booth to sell condoms. Another key focus is to remove inhibitions about contraceptives. I understand that in India also, people are reluctant to ask for a contraceptive in a drug store." At present, the belly tray is being used for promotions. "But we are also thinking of making it a full-fledged tool to sell condoms at least in countries like Nigeria, Cameroon and Mozambique where we are actively working," she says. Under the UAFC, various organisations have already tried ingenious tools for marketing condoms. They have inducted female hair dressers to sell the condoms as African women spend lot of time for their hair-do. "There is no barrier of communication about condoms in a female beauty parlour," she says.

Replace all sugar with honey: Cut out sugar from your diet. This means you will also have to give up on artificial sweeteners. Use honey in your tea, coffee and cereal instead of refined sugar. Keep a check on what you cook too, so that you don't use sugar in there either.
Skip junk food: Junk foods are processed food that contains empty calories. To fully benefit from the honey diet, stop eating junk food.
Opt for unrefined carbs: Refined white flour in white pasta and white rice may cause a spike in blood sugar levels. Opt for wholemeal flour instead as they are good for digestion and will keep you fuller for a longer time.

Blogger may face EQC court case costs |
The blogger who leaked thousands of confidential client details may have to pay the Earthquake Commission its court costs after the High Court found he was in breach of confidence. In early April, Marc Krieger, a former EQC employee, leaked a confidential spreadsheet detailing client and claim information for 83,000 EQC clients. He then published that information on a website called EQC truths. EQC brought a civil lawsuit against him for breach of confidence. In his decision, Justice Collins said he concluded that the information contained in the spreadsheet that Krieger published was confidential.
He said Krieger acknowledged that he received the spreadsheet from a "disgruntled EQC employee" and must have known that he was not authorised to receive it. That employee has never been identified. Justice Collins said Krieger believed he was acting with a clear conscience when he disclosed the information about the costs of repairing individual claims. However, he said those affected by EQC's decision who had a "legitimate interest" in knowing how their claims were being assessed by EQC had the option to apply for the information through the Official Information Act. "It was not for Mr Krieger to take it upon himself to tell the world at large about the details of individual claims." Justice Collins said the information also had "genuine" commercial interest that needed to be kept confidential to allow EQC to undertake its responsibilities in a "fiscally prudent manner".
Krieger had suggested that the information was in the public interest. Justice Collins said the real public interest was in preserving private information of those dealing with EQC. "Those who negotiate with or provide information to EQC should be able to do so freely and frankly, unhindered by the prospect that their private information will become publicly available." Limits on Krieger's freedom of expression only related to the contents of the spreadsheet and would not prevent him expressing opinions about EQC or the Government. Justice Collins granted a declaration that Krieger's disclosure of the information was a breach of his obligation to keep it confidential.
He also issued a permanent injunction on the information and opened the door for EQC to seek costs against Krieger. EQC chief executive Ian Simpson welcomed the court's decision.
